authentication hook to read data from mysql

Hi, i need help setting up the authentication hook to read data from the mysql database. I tried following the details in the doc section, and I can read the data from the database verify the login details and if successful create a cookie, which I am then supposed to pass to the hook.
But i am unable to understand how to pass the cookie, could you please write a short code to do the same,
1. Create the cookie after reading the data from mysql
2. Pass the cookie to the hook.
