prompt popup with kick/ban

I am attempting to create a prompt popup box so that an admin can input a kick/ban reason when using the contribution below:

I have attempted to modify the prompt box code from the new nick command and from the BBcode toolbar commands but I haven't had any luck. If someone out there has successfully created a prompt box with a new command I would appreciate it if you would post your code or if someone can point me in the right direction to create a prompt in phpfreechat that would be great. I am sure it is pretty simple by my javascript skills are lacking to say the least!
